Encore pharmacy knows the impact Crohn’s disease and Ulcerative Colitis have on a patient’s life. Through Encore's patient care monitoring program, our pharmacists and patient-care team regularly check in with patients to make sure that they stay on track with their treatment. In addition to helping with adherence issues, our pharmacists and patient care team monitor for improvement in Crohn’s and UC symptoms (e.g. abdominal pain, abscesses, fatigue, fever, mouth ulcers, nausea, etc.) as well as for overall health. The team inquires about Crohn’s Disease flares and monitors how the patient is doing overall in his or her everyday life through a quality of life screening using a clinically validated tool.
